Administer injections as requested by MD/NP.Work closely with CMO, Director of Operations, Clinical Nurse Manager and Practice Manager to ensure health center operates in coordinated, organized manner.Coordinate care and scheduling with Registered Nurse staff at other sites.Provide oversight and direction in scheduling and follow-up visits for patients as clinically indicated, including triage.Provide health education to patients and their families as needed.Prepare examination/treatment rooms; measure vital signs (i.E., pulse rate, temperature, blood pressure) as well as height and weight.Maintain/oversee inventory and order medical supplies and materials.Oversee and maintain log books for quality control.Preload/cue patient charts (med list, problems, allergies, vitals) prior to examination as needed.Clean and sterilize instruments as needed.Operate electronic electrocardiograph (EKG), and other equipment to administer routine diagnostic test or call a medical facility to schedule patients for tests as needed.Administer treatments and perform routine laboratory tests as needed.Utilize NYSIIS for immunization tracking/upload.Triage same day appointments or problem visits.

Pre-visit planning.ER/hospital discharge planning, care coordinator and transition of care.Assist Clinical Nurse Manager in recruitment and mentoring of nursing and clinical staff.Utilize Healthix to receive patient hospitalization, ER and rehab tests and procedures and preloads EMR.Complete environmental rounds and equipment inventory.Schedules bi-annual pharmacy inspection and equipment inspection, as requested by Clinical Nurse Manager.We require:Associate’s Degree or Bachelor’s Degree in nursing, preferred.Current NYS RN License.Strong computer, verbal and written communication and organization skills.Knowledge of Public Health practices/procedures, HRSA regulations, federal regulations and EMR.Bilingual; (English/Spanish) preferred.Flexibility to work in both health center locations as assigned.Registered Nurse (RN),for immediate consideration, please CALL/TEXT GORDON at 516-805-0667 or email garchibald@ahrc.org.an EOE.Advantage Care Health Centers is an FTCA Deemed Organization.
