Licensed Practical / Vocational Nurse A

Under the direction of an RN or other medical professional, provides general nursing care to patients.

Requires LPN or LVN license.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Other duties and responsibilities may be assigned.+ On Site daily+ Support residents in the (self) administration of medication+ Documentation of (self) administration of medication+ Performs TB and Drug screenings on all patients within 72 hours of admissions+ Performs routine urine drug screens as directed by the treatment/recovery plan+ Monitors and records vital signs+ Provides triage and provides medical services accordingly+ For patients with health care needs, schedules and coordinates all medical appointments, and provides periodic follow-up+ Reviews and transcribes medication orders, and monitors medication records for compliance;+ Observes and records response to treatment/medication and notifies the Medical Director, the RN Supervisor, and the Program Director of any such instances+ Monitors ongoing/chronic medical problems+ Handles minor needs of patients including attending to minor injuries, administering routine medications+ Maintains all medical records+ Performs QA activities+ EKG+ Performs accurate and complete audits of medication transported, received, administered, stored or disposed, as trained and assigned, in accordance with Phoenix House medication policies and procedures.COMPETENCIES (KSAs)+ Ethical Practices+ Judgment + Relationship Management+ Time Management+ Empathy+ Professional Boundaries+ Global and Cultural Awareness+ Critical Evaluation+ Problem Solving+ Decision Making+ Communication+ Teamwork + Crisis Management+ Technical CapacityREQUIRED EDUCATION+ Graduate from an accredited nursing programLICENSES/CERTIFICATIONS+ Current NYS license+ Current certification in CPR, Child Abuse and Infection ControlREQUIRED EXPERIENCE+ At least three years related experience+ Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugsWORK ENVIRONMENT+ This job operates in an indoor medical clinical environment.

This role also routinely comes into contact with patients who may have contagious illnesses.
