Breast Cancer Nurse Navigator

JOB TITLE: Breast Program Patient Navigator STATUS: Exempt

GENERAL STATEMENT OF DUTIES: The breast navigator’s primary function is guiding breast cancer patients through the health care system by assisting with access issues, developing relationship with physicians and other service providers, and tracking interventions and outcomes, along with patient and caregiver education.


TYPICAL PHYSICAL DEMANDS: Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye hand coordination.

Requires standing and walking for extensive periods of time.

Occasionally lifts and carries items weighing up to 100 pounds.

Requires corrected vision and hearing to normal range.

Requires working under stressful conditions or working irregular hours.

Requires exposure to communicable diseases or bodily fluids.

TYPICAL WORKING CONDITIONS: Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.


  • Work in conjunction with the BCP nurse navigators in the continuum of care form surgeon or gynecology office to HOA medical oncology and radiation oncology services.
  • Guide patients through the health care system offering easy access to care and to the breast navigator.
  • Connect patients to social work, dietician, mentor program, wellness center, survivorship program, and other support agencies.
  • Assist patients in understanding their diagnosis and treatment options through reinforced education.
  • Track and follow up with HOA breast cancer patients during and after chemo/biotherapy, including oral therapies.
  • Call patients frequently and offer education and symptom management along with coordinating on treatment follow up appointments.
  • Reach out to patients in symptom follow up.

    Coordinate impromptu office visit when deemed clinically necessary for sick visit and or follow up on patient concerns.

  • Document navigation interventions and suggestions and communicate to HOA MD and care teams.
  • Track breast patient HOA orders specifically outside referrals and outcomes.
  • Track new pathology findings and radiology finding, document in the EMR and update disease status in the EMR according to new findings.
  • Develop patent education programs and tools.

    Maintain a learning library in patient specific languages.

  • Assess breast patients for genetic risk along with tracking previous genetic testing.
  • Referring for eligible breast patients for genetic testing.
  • Provide coordination as needed in obtaining insurance and/or forms required for financial and other types of assistance.
  • Maintain open communication with providers, caregivers, and navigated patients in order to coordinate services within the organization, with external facilities, and within the community.
  • Assist in increasing access to culturally appropriate, supportive care when and where possible and refer patients to appropriate sources for needed items (wheel chairs, dressings etc.).
  • Streamline appointments, documentation and paperwork.
  • Present to HOA clinical teams and physician’s updates and process changes.
  • Track interventions and quality outcomes with navigation metrics.
  • Cross cover with the thoracic and GI navigators.
  • Evaluate whether or not the navigation program is meeting its purpose and suggest quality improvements where necessary.
  • Implement strategies to promote awareness of the navigation program in the organization and the community.

Breast Program Navigator Outreach Activities Include :

  • Work with BCP nurse navigator in community groups to increase breast health awareness, and involve the community in program planning if possible.
  • Assist BCP nurse navigators in conducting breast health education classes.

Basic Knowledge, Skills and Education Requirements :

  • The ideal candidate should be a registered nurse with 3 or more years of experience in medical surgical nursing and oncology.
  • BSN and OCN certified.
  • The Breast cancer navigator must have outstanding interpersonal, oral and written communication, organizational and multi-tasking skills.

    The individual must poses knowledge of breast cancer, breast cancer treatment modalities, community resources and healthcare structure and function.

  • Understanding of breast cancer risk factors and genetic mutations.
