Registered Nurse (Health Care Coordinator)

This is a Full-time position in Valley Cottage, NY posted June 27, 2021.

Job description

Position Summary (main purpose and function):

• Provide medical oversight to clients taking part in an Opioid Treatment Program (OTP).

• Provide support to Nursing Supervisor by executing duties within medical compliance to regulatory standards.

Position Responsibilities:

• Meet with Client to complete the following elements of initial Nursing intake:  MMTP Patient Medical

Orientation form, list of medications, allergies, referral for HIV and STI testing, give intake packet (explaining medication-assisted treatment and what to except)

• Obtain medical records from various providers as required for coordination of care

• Keep and maintain completed correct records of clients with communicable disease and treatments

• Meet with client post-admission monthly for first three (3) months and annually; address any medical issues and refer as necessary

• Met with all clients annually approximately one (1) month prior to anniversary of earlier physical to obtain annual blood work, PPD and U/A

• Ensure all clients are up to date on annual physical and six (6) month review; schedule their appointment with the Physician in 10e11and keep complete and correct record

• Register with HealtheConnections to check daily for clients that have been/are admitted to acute care medical facilities; provide follow-up as needed with Prescribers and primary Counselor

• Meet with client upon returning from inpatient hospitalization.

Obtain vital signs, copies of discharge paperwork, confirm last dose (of methadone) with hospital pharmacy and document such in client ECR

• Register with Lab portal Bio-Reference to check daily for earlier labs, and bring attention to provider of abnormal values

• Meet with pregnant clients on a bi-monthly basis.

Confirm pregnancy, consent for OB on file and refer to OB if needed.

Explain the Department of Health’s role and services that are available and offer referral.

Provide support and education on NAS, delivery process, NICU, breastfeeding, morning sickness and vomiting, prenatal vitamins, CPS, WIC referral.

Drug use during pregnancy and effects on unborn fetus.

Contact OB if client has positive toxicology.

Educate on birth control option for future.

Referral for abortion services as necessary; maintain accurate records

• Supply smoking cessation education, support and referrals as needed

• Refer all clients with Hepatitis to primary MD or GI provider, health home for hepatitis evaluation and treatment; keep records of such

• Refer clients to their local Department of Health for vaccines against Hep A+B, Varicella, Pneumonia

• Complete Department of Health communicable disease form (as forms are requested) with client and fax back in a timely manner

• Obtain and supply safe sex products from NYS Condom program.

Educate on importance of safe sex practices

• Educate and supply written materials on harm reduction services local to the area.

SEP, safe sex packs, bleach kits (Project Reach out, HVCS)

• Provide and keep correct records of staff annual PPD testing and/or chest x-rays as per COA regulations

• Ensure client care is provided in accordance with internal policy and procedures as well as external regulations i.e.


• Recognize, treat and report changes in the client’s physical and emotional condition related to MAT services; assess and respond to medical emergencies as they arise

• Provide proper linkages, referral, coordination and follow-up for client to services and supports including referrals and linkages to long term services and supports

• Ensure releases of information and other required documents are completed and filed in client ECR

• Responsible for scanning all medical documentation received from other providers into client ECR, along with any medical documentation you have generated that pertains to the client into their ECR

• Work closely with community resources and organizations in coordinating client services

• Participates and supplies training and in-service education

• Participate in ongoing supervision, staff meetings and training

• Always maintain confidentiality in compliance with HIPPA standards

Skills, Knowledge, Education, Experience:

• State Nurse Licensure required

• Two (2) years prior experience working in a health care related facility

• Prior experience working in field of alcohol/substance abuse treatment preferred

• Prior counseling or teaching experience is required

• Narcan trained, TOT trained

• CPR for health care compliant

Working Relationships (internal & external)

• Nurse must keep open communication with clinical colleagues (all Prescriber staff, Nurses, Therapists,

Consultants, Counselors, Case Managers)

• Nurse needs to keep open communication with external service providers as needed (i.e.

DSS, Department of Health, schools, health services providers, pharmacy, labs, etc.)

Physical Demands:

• Potential exposure to bodily fluids (i.e.

urine samples, blood, oral swabs) Use of universal precaution and personal protective equipment (latex gloves) is required (when on-site).

Work Environment:

• When on-site: office environment conducts business of a sensitive and confidential nature.

May be working with or near minors
