Our mission is to support individuals, who otherwise qualify for nursing home placement, to remain as independent in the community for as long as possible, delaying or avoiding placement. Our Rotterdam team is supporting approximately 210 participants in the community on average.We provide a team-based model of care including a physician, NPs, RNs, LPNs, Home Health Aides, social workers, physical/ occupation/speech therapists, transportation, a day center team, and more! This is an opportunity to be on the front-line of the future of healthcare delivery.Responsibilities:* The primary purpose of this job position is to provide and supervise the day to day in-home nursing activities of Eddy SeniorCare* Such supervision must be in accordance with current Federal, State and local standards, guidelines, and regulations that govern PACE to ensure that the highest degree of quality care and outcomes are maintained at all times* Supervise RN Supervisors: our RN Supervisor for our Primary Care Clinic and our RN Supervisor for our Home Care Team of aides. The RN Manager also supervises our Intake RN, and our team of RN Case Managers, LPNs and RN Home Care Visit nurses* Actively participate in planning and directing of day to day operations to achieve our mission and goals* Oversee staff in use of supplies and equipment for effective and safe responsible useWhat you will need:Required:* A current license to practice as a Registered Nurse (RN) in the State of New York* Bachelor’s Degree of nursing* 5 years or more of recent experience as a RN ideally in home care, ED, ICU or oncology* 3 years or more of experience in a leadership role* Valid driver license and reliable vehiclePreferred:* 2 years or more of experience in a home care setting* Prior Case management experience* Must possess a strong passion for caring for seniors and demonstrate a respectful, caring, professional demeanor* Excellent time management and organizational skills* Highest level to communicate effectively, verbally and written and possess strong critical thinking skills to problem solve* Can de-escalate and manage staff and participant concerns in a caring manorTrinity Health’s Commitment to Diversity and InclusionTrinity Health employs about 133,000 colleagues at dozens of hospitals and hundreds of health centers in 22 states.
