Make a bigger difference nn PURPOSE STATEMENT:Part Time Social Worker: 21 hours per week nn nn The team’s purpose is to consistently provide the highest quality care possible to Jewish Board clients regardless of age, diagnosis, race, or ethnicity that meets agency and healthcare professional standards, reflects our mission while also maintaining collaborative partnerships with community stakeholders.

nn nn nn nn PROGRAM: nn nn The Children’s Mobile Crisis Team (CMCT) provides rapid response services to children age 0 to 17 experiencing acute mental health crises in the Staten Island Community.

The Children’s Mobile Crisis Team is designed to reduce 911 calls as well as the number of children referred to psychiatric emergency rooms.

nn nn nn nn OVERVIEW: nn nn The Social Worker is responsible for supplying necessary clinical interventions within the realm of the Rapid Response Mobile Crisis program.

Provide cultural competent and sensitive services.

As part of a therapeutic team within the Agency, work closely with the divisions’ medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client.

nn nn Flexibility  schedule around shift is required.

  nn nn RESPONSIBILITIES include but not limited to: nn nn
•         Coordinate all services for consumers, including day programs, behavioral health, medical care, transportation, summer programs (when applicable), recreational activities, and community inclusion (Person Centered Planning).

•         Liaise between Department and consumer’s family members, and relevant community members.

nn nn
•         Maintain records for all contacts with clients and their families.

nn nn
•         Link clients to community resources that support their behavioral health goals.

nn nn
•         Conduct meetings to ensure proper treatment, progress in program and coordination of goals.

nn nn
•         Participate in audit preparation.

nn nn
•         Work with program supervisor on intake process for new consumers.

nn nn
•         Work with Family Advocates to enhance communication and cooperation between them and consumer families.

nn nn
•         Complete paperwork in a timely manner.

nn nn
•         Other duties as assigned.
