Panel Management Coordinator

This is a Contract position in Island Park, NY posted January 10, 2022.

SUMMARY: Under supervision of the Health Outcomes and Outreach supervisor or designee, collaborate with a multidisciplinary team to improve health outcomes of patients through population health and panel management strategies.

Primary responsibilities of this position include assisting in the improvement of the quality of preventive and chronic care services, assisting the medical team in preventive care and chronic disease management, managing health information data through the use of disease registries, and participating in continuous quality improvement activities and providing health education.

Assist Ambulatory Care leadership in identifying population management priorities and interventions.2.

Collaborate with assigned healthcare team(s) to create, implement and evaluate population management interventions; meet regularly to ensure optimal process and outcome deliverables.3.

Collaborate with clinic leadership to engage staff in site level quality data4.

Collaborate with Clinical Data Coordinator and the quality team to enter data into and maintain registries.5.

Conduct database reporting as requested to support site specific quality initiatives.6.

Ensure integrity of patient registry and empanelment data through periodic audits and regular updating of information.7.

Order health maintenance items per protocol to support gap in care outreach to patients.8.

Perform multimodal out-reach and in-reach population management/health coach activities as assigned.9.

Provide appropriate health education and health coaching interventions as appropriate.MINIMUM QUALIFICATIONS:Education: High school diploma or GED and some college coursework in healthcare, health promotion and education, or related field.Minimum Experience: At least one year of providing direct patient service in a clinical or public health setting is preferred; or equivalent combination of education and experience.

Experience in adult education and/or group facilitation.Minimum Experience: Bilingual/bicultural in Spanish or Asian languages is highly preferred.Required Licenses/Certifications: Acquire and maintain active Medical Assistant certification as issued by American Association of Medical Assistants (AAMA), California Certified Medical Assistant (CCMA), National Center for Competency, Testing/Multi-skilled Medical Certification Institute (NCCT/MMCI), or American Medical Technologists (AMT); BLS
– Basic Life Support Certification issued by the American Heart Association.Additional InformationMust have reports experience with EPIC and must be certified in medical assisting.
