Patient Navigator (Spanish Bilingual)

This is a Contract position in Sodus, NY posted November 28, 2020.

This position will float to various FLCH sites, with our brand new facility in Sodus, NY being the home base!
General Purpose: Responsible for facilitating communication and coordinating care among patients, family members, community, and healthcare providers to promote better health outcomes.
Main Responsibilities:
1. Responsible for assisting the navigation of patients through healthcare appointments:
a. Assists patients completing paperwork, such as Cancer Services Program, Financial Aid, Prescription Assistance, Intake paperwork etc.
b. Assures that patient flow is smooth, attends daily huddles
c. Interprets for patients,
d. Actively participates in pre-visit
e. Assists with follow up on patient no shows,
f. In an effort to reduce no shows and barriers to care, analyses high-risk patients, and addresses barriers to care and Social Determinants of Health
2. Responsible for performing various clerical duties:
a. Maintains/works on registries as indicated per program
b. Assists with scheduling, and rescheduling patient appointments for medical, dental, interpretation etc.
c. Adds and completes appointments on Resource schedule, and documents patient encounters on eCW.
d. Promptly responds to all referrals, Telephone Encounters, and Actions assigned by a healthcare provider, according to FLCH policy.
e. Maintains program brochures and community resources inventory, responsible for updating resources when necessary.
f. Maintains a Tele dental Caseload, responsible for following families and adhering to workflow protocol
3. Responsible for empowering patients by providing current information and education:
a. Refers patients to appropriate agencies as necessary.
b. Advocates for patients, helping them access resources and services as needed
c. Performs duties as assigned by supervisor

General Development:
a. Attends staff trainings
b. Participates in trainings as directed by supervisor
c. Actively participates in monthly/bi-monthly team meetings
Education and Certifications:
a. High School or Equivalency Diploma
b. Interpreter certification
Communication Skills:
a. Must possess the ability to communicate fluently in Spanish and English
b. Must have neat and legible handwriting
c. Must be able to interact with patients courteously and calmly
d. Ability to communicate well with the public
